Initial Contact
Get in touch via our referral form, email, or phone. Tell us about the person and their needs.
Assessment
We arrange a comprehensive needs assessment, meeting the individual and their family or support network.
Transition Planning
We create a personalised transition plan, including visits, meet-the-team sessions, and gradual introductions.
Moving In
The individual moves into their own bedroom at their own pace, with dedicated 24/7 support to help them settle into their new home.

Funding Information
We work with various funding arrangements and can advise on options available.
Local Authority Funded
The most common funding route. Your local authority carries out a Care Act assessment and arranges a support package based on assessed eligible needs. In supported living, housing costs are funded separately via Housing Benefit.
NHS / CHC Funded
For individuals with a primary health need, NHS Continuing Healthcare (CHC) may fund the full cost of the support package.
Self-Funded
Individuals or families who wish to arrange and fund their own support are welcome. We offer transparent pricing and flexible support packages.
